The post-holder will work together with consultant haematologists and anticoagulation CNS to provide and develop the anticoagulation and haematology service at Queen Elizabeth Hospital. This post is a permanent post, and is an excellent opportunity for a clinical pharmacist, with experience working in anticoagulation, and a desire to develop their leadership and management skills. You will be expected to lead and develop the both clinical haematology & specialist anticoagulation services across the Trust. This includes the promotion of safe, effective and cost effective anticoagulants use to facilitate patient admission and enhance continuity of care across the primary and secondary care interface. About us Job Summary 1. To lead, deliver, develop and evaluate clinical pharmacy services relating to anticoagulation across the Trust, including Promote safe and effective use of anticoagulants during patient admission. Enhance continuity of care across the primary and secondary care interface. To facilitate and support the safe, efficient and cost-effective use of medicines used within the anticoagulation service. To support the anticoagulation services through direct clinical care commitments for prescribing and counselling at the designated anticoagulation clinics and provision of expertise at the relevant specialist MDTs. To support the implementation of the National Commissioning Policy for immunoglobulins within the Trust, including participation in the local and sub regional panels. 2. Represent the Pharmacy department at the Thrombosis Committee and the Trust at the Immunoglobulin Assessment Panel and South London Cardiovascular Network. 3. Act as a role model and provide a consistently high standard of pharmaceutical service to patients. 4. Support divisions across the Trust in managing anticoagulant expenditure and act as a liaison for service developments involving anticoagulation services. 5. Provide operational and clinical support to the pharmacy department as needed, including supervision, training, and support to junior pharmacists and other pharmacy staff. 6. Deputise for the Principal Pharmacist for Surgery and Theatres at Surgery Divisional meetings if appropriate. Key Results Areas & Performance: Clinical Services and Patient Care 1. Participate in the front-line provision of Pharmacy Services as required. 2. Act as a clinical pharmacist in an appropriate area and participate in consultant ward rounds. 3. Serve as a clinical role model, ensuring safe, clinically effective, and cost-efficient use of medicines. 4. Lead and develop the specialist anticoagulation services, including pharmacy-led anticoagulation clinics. 5. Coordinate training and assessment of new pharmacists in anticoagulation clinics. 6. Train pharmacists on anticoagulant use to ensure safe clinical pharmacy service provision. 7. Provide a safe and patient-focused clinical pharmacy service, including medicines reconciliation and facilitating the use of patients own drugs. 8. To be an active member of the inpatient pharmacy team, with a focus on anticoagulation. To provide leadership support in surgery when needed. 9. Be accredited for discharge prescription transcribing/drug listing. 10. Support the investigation of medication-related incidents and develop action plans. 11. Contribute to strategies for improving safe medicines use across the Trust. 12. Liaise with primary care colleagues on patient medication issues. Leadership & Service Development 1. Lead the appropriate use of anticoagulants, setting and monitoring quality standards. 2. Act as pharmacy representative and lead service developments within anticoagulation and IVIg. 3. To contribute to the development of the Trust anticoagulation stewardship plan 4. Provide the Clinical Director of Pharmacy with performance output at specified intervals. 5. Identify resources needed to improve pharmacy services. 6. Support adherence to local and national guidelines for safe prescribing of anticoagulants. 7. Collaborate on Integrated Care Pathways and Patient Group Directions for services. 8. Liaise with other senior pharmacy staff for effective service provision. Division Support 1. Contribute to managing anticoagulant expenditure through monitoring and audits. 2. Identify potential cost-saving areas and implement systems for savings. 3. Educate healthcare professionals on the costs associated with medications. 4. Participate in business case preparation for service developments. 5. Actively participate in the Thrombosis Committee and division meetings. Staff Management and Training 1. Manage the induction and development of pharmacy staff. 2. Conduct regular performance reviews following Trust procedures. 3. Support the education and training of other healthcare professionals. 4. Participate in clinical training programs at local and national levels. 5. Evaluate and develop management and professional knowledge competencies. 6. Undertake continuing professional development (CPD). 7. Arrange departmental meetings to update staff on current issues. 8. Serve as a postgraduate tutor for pharmacy staff as needed. Research & Development 1. Lead audits and practice research to improve service quality. 2. Encourage research publication and presentation, including participation in the Associate Principal Investigator Scheme. 3. Develop collaborations with academic units for pharmacy practice research. Governance, Information & Data Management 1. Develop performance measures and disseminate information. 2. Lead on review of audit data related to pharmacy service outcomes. 3. Support investigations of medication-related safety incidents. 4. Promote effective prescribing following formulary and NICE guidance. 5.
