District nurses make a difference everyday to the lives of the people they visit at home and in residential care homes. They provide increasingly complex care for patients and support for family members. You will visit patients in their own homes and provide a comprehensive range of clinical nursing activities. A key aspect of the role will involve working with the wider multidisciplinary team to help plan and deliver complex care to enable patients to remain in their home environment for as long as possible. Working as part of an integrated locality team, you will work in partnership to reduce hospital admission and facilitate discharge by providing clinical holistic assessment and treatment. You will promote optimal levels of independence and wellbeing for patients whilst facilitating teaching and support for formal and informal carers. About us Applicants must have a full valid driving licence and have access to a car for work. If necessary, adjustments can be considered in line with the Equality Act 2010. The service operates 7 days per week, 8am to 6.30pm. Full or part time (minimum 22.5 hours) roles are available.
