If the answer is yes, then we want you to work with us.

Community nursing is a challenging and rewarding career, providing care for patients within their own homes and the community setting.

District nurses make a difference everyday to the lives of the people they visit at home and in residential care homes. They provide increasingly complex care for patients and support for family members.

You will visit patients in their own homes and provide a comprehensive range of clinical nursing activities. Working as part of an integrated locality team, you will work in partnership to reduce hospital admission and facilitate discharge by providing clinical holistic assessment and treatment.

You will promote optimal levels of independence and wellbeing for patients whilst facilitating teaching and support for formal and informal carers.
