The post holder will support the domiciliary oxygen service team, ensuring a smooth and efficient service for patients throughout their pathway and follow up care. They will assist in the day to day co-ordination of the service, monitor and instigate reviews both in the outpatients department and in the patients homes, (using own transport). They will also help to analyse data provided by the ICB or Oxygen provider and raise any queries/or highlight any concerns to the trained nurse.The post holder must be able to use their own initiative and work autonomously, but also work proactively withinthe team to ensure the service runs to maximum efficiency. About us To demonstrate a good knowledge and understanding of the oxygen assessmentprocess, oxygen equipment and treatment options. To communicate with patients,carers and professionals on all levels on a daily basis with regards to current oxygen prescriptions and equipment. Engage with domicillary oxygen provider regarding patient queries orders or discrepancies To ensure the oxygen database is kept current and up to date. To prioritise patient safety by completing risk assessments within the patients own home 4 weeks post discharge, (in line with 4 week post discharge guidelines) or visit patients own home and complete a risk assessment prior to discharge of high risk patients. Be confident in knowledge and understanding of domicillary oxygen equipment, to enable competent discussions/ advice to patients experiencing issues. To complete all clinical records accurately and legibly as instructed. Ensure timely return of investigations/reports as requested by Oxygen team and be aware when immediate escalation is required To report untoward occurrences immediately to senior members of the team. To be empathetic to patients medical and psychological needs when encouraging and implementing or changing oxygen therapy regime. To be able to advise patients who wish to travel, on ordering oxygen both within the UK and overseas Participate in multidisciplinary team meetings. Undertake other duties appropriate to the post as requested by senior staff. "}
