The post is integral to the multidisciplinary team, working closely with our partners in secondary care to provide seamless transition for patients to primary care underpinned by a solid clinical governance structure. The post holder will build relationships with wider external organisations to promote joined up patient care and services. Responsible for the assessment, planning and implementation of clinical management plans. Deliver direct care to patients including providing personalised planned interventions to support people to remain safe, well and as independent as possible in their own homes. Initiate insulin and proactively monitor the care of all diabetic patients in line with local and national policy. Provide education and training to health care professionals across the pathway and developing the focal point for responsive, high quality, evidence based clinical advice in the out of hospital system, closely integrated with secondary care specialities. Works in partnership with other groups in the development of innovative clinical practice. Assists in managing a caseload, in line with practice policies, guidelines and national standards. Promotes and enables patient choice and independent living. Working within a structured diabetic team currently at the practice. Adjustment of insulin as well as initiation. Addition/ initiation of diabetic drugs including Metformin, GLP agonists, SGLT2 inhibitors and oral hypoglycaemic agents. Support of childhood diabetics and liaison with secondary care Paediatrics teams who manage these clientele.
