To support our practice and ensure patients receive the best quality, Millgate Healthcare Partnership is seeking an experienced Diabetic Nurse to work alongside our existing nursing and GP team to develop and deliver a comprehensive diabetic service for our patients.
To be responsible for the management of diabetes including insulin initiation, health education, reviewing bloods, medication monitoring and dose adjustment. Working as part of the nursing team, delivering care within their scope of practice to the entitled patient population. The role will include supporting the management team in the reviewing of clinical policy and procedure.
The post holder will deliver clinic and home-based treatment plans including education and lifestyle advice, monitoring and management of symptoms and medications.
The service seeks to provide access to diagnostics and early treatment intervention. The post is integral to the multidisciplinary team, working closely with our partners in secondary care to provide seamless transition for patients to primary care underpinned by a solid clinical governance structure. The post holder will build relationships with wider external organisations to promote joined up patient care and services.
