Manage inbound referrals and respond appropriately to patient and family queries. Update patient records with care plans during MDT meetings. Coordinate follow-ups with the clinical team and liaise with hospital teams and practices regarding patient care. Process patient discharges and ensure all care-related tasks are completed. Report incidents, complaints, and escalations to the Service Manager. Perform additional duties in line with NHS policies and legislative requirements as necessary.
