The Service Coordinator role with Greater Peterborough Network involves coordinating high-quality and effective patient care through collaboration with healthcare teams while fostering a positive working culture. Key responsibilities include managing inbound referrals, addressing patient and family queries, updating patient records during multidisciplinary team meetings, coordinating follow-ups with clinical teams, liaising with hospitals and practices regarding patient care, processing patient discharges, and reporting incidents or complaints to the Service Manager. This position emphasises the importance of effective communication within a supportive environment, allowing for both professional growth and meaningful contributions to service improvement.
