Deliver high-quality, safe, and effective patient care as part of a collaborative team. Promote a positive working culture by encouraging staff participation in service development and improvement. Manage inbound referrals and respond appropriately to patient and family queries. Update patient records with care plans during MDT meetings. Coordinate follow-ups with the clinical team and liaise with hospital teams and practices regarding patient care. Process patient discharges and ensure all care-related tasks are completed. Report incidents, complaints, and escalations to the Service Manager. Perform additional duties in line with NHS policies and legislative requirements as necessary.
