You will be working for Shropshire Community Health NHS Trust working cross-site at The Royal Shrewsbury Hospital and The Princess Royal Hospital, Telford. This is an exciting opportunity to make a difference to the care of patients enabling them to step down to their normal place of residence faster As a registered Occupational Therapist you will be experienced in working in a multi disciplinary / multi-agency health / social care environment with knowledge of IT basic administration systems including Microsoft Office Suite (Word, Excel etc). You will be patient focussed and have the ability to carry out comprehensive assessments and be able to prioritise and make sound decisions, sometimes under pressure. To provide holistic and person-centred care and support community patients in a subacute setting. Responsible for the assessment, planning, delivery and evaluation of individualised patient care, working in partnership with patients, their carers and the multi-disciplinary team to ensure the patient's physical, spiritual and emotional needs are met. To complete Occupational Therapy assessments of patients with diverse presentations, with complex physical and psychological diagnoses, to inform treatment planning based around agreed and realistic goals.
