Band 6: £37,338 to £44,962 a year per annum  PERMANENT  GOOD SALARY

You will be working for Shropshire Community Health NHS Trust working cross-site at The Royal Shrewsbury Hospital and The Princess Royal Hospital, Telford.

This is an exciting opportunity to make a difference to the care of patients enabling them to step down to their normal place of residence faster

As a registered Occupational Therapist you will be experienced in working in a multi disciplinary / multi-agency health / social care environment with knowledge of IT basic administration systems including Microsoft Office Suite (Word, Excel etc).

  • You will be patient focussed and have the ability to carry out comprehensive assessments and be able to prioritise and make sound decisions, sometimes under pressure.
  • To provide holistic and person-centred care and support community patients in a subacute setting.
  • Responsible for the assessment, planning, delivery and evaluation of individualised patient care, working in partnership with patients, their carers and the multi-disciplinary team to ensure the patient's physical, spiritual and emotional needs are met.
  • To complete Occupational Therapy assessments of patients with diverse presentations, with complex physical and psychological diagnoses, to inform treatment planning based around agreed and realistic goals.
