As a Senior Clinical Assessor/Practitioner (Registered Nurse or Allied Health Professional) in Continuing Healthcare, you will be responsible for conducting comprehensive individual assessments of patients to assess their eligibility for NHS Continuing Healthcare (CHC) to enable timely provision of care, case managing clients to ensure that their needs are effectively met and promoting Personal Health Budgets and personalisation to empower people with complex health needs live their best lives. You will need to have excellent interpersonal, presentation and communication skills and an ability to make decisions in a challenging but supportive environment. As part of the role, you will have the opportunity to develop strong links with health and social care colleagues to ensure effective multi-disciplinary working and develop a strong understanding of local NHS and independent sector service provision in individual's own homes and in care homes.
