Band 6: £37,338 to £44,962 a year per annum  PERMANENT 
Whilst experience in Continuing Healthcare would be helpful, if you have the right skills and attitude, we can support you to develop the specific skills required for Continuing Healthcare.

You will be highly organised, flexible and have a positive can do attitude.

Be highly motivated and keen to learn/develop (training in respect of the NHS CHC Framework and its application will be provided as part of induction).

Be accustomed to a high degree of autonomy on a day-to-day basis.

Be able to assimilate large amounts of information and have excellent verbal and written communication skills, with attention to detail.

Ability to develop relationships and foster effective networks with key stakeholders, families and carers and have developed people management skills with experience of resolving difficult situations and challenging individuals effectively and calmly.

Be comfortable working with a wide variety of ages and individuals with very different clinical conditions.

Be able to assess and prescribe care dependent on the individuals holistic health and care needs.

Have a strong value set around quality and patient/carer experience.

Have excellent time management skills and thrive on working at pace.

Have excellent knowledge of Microsoft office and have intermediate typing skills.

Be able to travel daily across the county and occasionally countrywide.

As a Senior Clinical Assessor/Practitioner (Registered Nurse or Allied Health Professional) in Continuing Healthcare, you will be responsible for conducting comprehensive individual assessments of patients to assess their eligibility for NHS Continuing Healthcare (CHC) to enable timely provision of care, case managing clients to ensure that their needs are effectively met and promoting Personal Health Budgets and personalisation to empower people with complex health needs live their best lives.

You will need to have excellent interpersonal, presentation and communication skills and an ability to make decisions in a challenging but supportive environment. As part of the role, you will have the opportunity to develop strong links with health and social care colleagues to ensure effective multi-disciplinary working and develop a strong understanding of local NHS and independent sector service provision in individual's own homes and in care homes.
