You will play a pivotal role to ensure the on-going effective delivery of Discharge to Assess (D2A) bed based services with key responsibility for the assessment and tracking of patients within the D2A service within agreed timeframes relieving the pressures to the Acute Hospitals. Working closely with multi-disciplinary teams, service users and relatives to perform early review of service users transferred into assessment beds to establish needs and funding requirements for discharge which will involve close working with health and social care staff. You will have an understanding of Knowledge of the Care Act 2014 and the principles of Discharge to Assess (D2A), National Framework for NHS Continuing Healthcare and NHS funded Nursing Care (Revised July 2022). The Service operates 5 days a week predominately with the nursing team working 8am-4pm however; as professionals, you will manage your own diary and timekeeping which may provide some degree of flexibility. You will be based at home but must be able to travel over the D2A footprint across North and South Staffordshire including but not limited to Cannock and Walsall. As a Band 6 NMC Registered Complex Assessment Nurse you will be required to prioritise your own workload to ensure effectiveness and efficiency in delivering objectives whilst working with minimal supervision. Being proactive and able to demonstrate good organisational skills and excellent communication skills both in verbal and written correspondence, overcoming barriers of service provisions and understanding The role is varied and requires confidentiality, initiative, flexibility and capability of working under pressure and to deadlines. As part of this role you will be expected to: Be responsible for attending Multi-Disciplinary meetings. Gather information from Care Plans and Professionals. Completing Assessments Complete Safe and Well Checks, Providing 1-1 reviews Providing Nurse Checks on new patients.
