Do you have a passion for working within adult mental health? Then come and join the team and meet likeminded people. The aim of the team is to bridge the gap between primary and secondary mental health care: providing accessible primary care level mental health interventions to the population of North Lincolnshire. The pathways within Primary Care Network Mental Health are: Routine Pathway, Dementia Pathway, Medication Management Pathway, Mental Health and Wellbeing Pathway, and Severe Mental Illness Physical Health Check pathway. These vacancy is within the routine pathway, but we are an integrated team, so awareness of all pathways is required. The routine pathway accept all routine referrals into mental health services. The role involves triage, assessment, signposting and offering brief interventions in line with the primary care model. The post holder will be expected to assess risk and ensure any urgent referrals are actioned by the relevant team. The North Lincolnshire Primary Care Network Mental Health are committed to establishing our service within the wider Primary Care Networks and have colleagues embedded in GP surgeries. Expanding this is the focus of our service. We welcome applicants who feel they have the drive, skills and expertise to continue this further development. The post holder will be an enthusiastic, highly organised and motivated mental health professional who can provide high quality nursing expertise and support within the routine pathway. It is essential that the applicant can demonstrate post registration experience as an RMN, which may include inpatient and community settings. The following tasks and duties will also form part of the role, however this list is not exhaustive: Triage of incoming referrals to establish urgency / risk Holistic mental health assessment Development of appropriate treatment plans / signposting to relevant third sector organisations Recruitment, supervision, and deployment of staff. Advice, training and induction and mentorship of learners. Audit, research and service monitoring.
