The Community Mental Health Nurse role with Rotherham Doncaster and South Humber NHS Foundation Trust in Scunthorpe is focused on providing accessible mental health interventions within the Primary Care Network. The position is part of an integrated team aimed at bridging the gap between primary and secondary mental health care, especially within the routine pathway that manages incoming referrals. Key responsibilities include triage, risk assessment, holistic mental health evaluations, developing treatment plans, and offering brief interventions. The successful candidate will work collaboratively with a multidisciplinary team and be involved in mentoring, supervision, and service monitoring.

Candidates should be experienced Registered Mental Health Nurses (RMNs) with post-registration experience in inpatient and community settings. A strong commitment to enhancing mental health services in North Lincolnshire, along with organisational skills, enthusiasm, and a passion for adult mental health, is essential. The role also emphasises the importance of establishing strong connections with local GP surgeries and third sector organisations to better serve the community’s mental health needs.
