Delivered by a dedicated caring team in co-operation with colleagues in other health and social care agencies. We are a busy Practice situated in the heart of South Shields. Our team is made up of 3 GP Partners, 1 Salaried GP, an Advanced Nurse Practitioner, a Practice Nurse, a Health Care Assistant and a full complement of administrative staff. We are looking to recruit a Practice Nurse for 24 hours to enhance our current team and to provide exemplary patient-focussed, high-quality care. The main duties of this role include general practice nursing duties (including but not exclusive to) Chronic Disease Management Wound Care Immunisations (routine and travel for adults and children) Cervical Screening Injections Ear checks/care About us Please see attached full Job Description which includes: Accountability You are ultimately accountable to the Partners but the day to day running of the Nursing Area is the responsibility of the Practice Manager with the Advanced Nurse Practitioner as peer support. Main Purpose of the Post Responsible for providing a comprehensive range of nursing care in the GP practice including the development, implementation and evaluation of care and/or treatment programs for patients with established long-term conditions, working independently in accordance with practice protocols. Provides a wide range of general clinical care and chronic disease management reviews to practice population through the Year of Care initiative. The Post holder will show total respect and regard to confidentiality of patient information at all times. The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative health care to the practice population. The Practice Nurse is responsible for the care delivered, demonstrating skills of assessment and decision making in the day-to-day management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures. The ethos of the Practice is to promote healthy lifestyle, provide health education and deliver good quality primary healthcare to all our patients within the surgery and the home environment identifying and addressing their health needs. Key Duties Advise and counsel patients Working to protocols and provide care accordingly To give holistic assessment of patients needs and to give advice and treatment Undertake screening/health promotion procedures Running of Special Clinics using approved protocols, for example, Year of care reviews incorporating multiple chronic diseases including CHD, Stroke etc Hypertension Asthma/COPD Contraception reviews HRT/Menopause reviews Well Woman Also to be able to, Administer travel and routine immunisations to babies and adults Initiate investigation, treatment and patient management where appropriate Read and action results sent from the lab forwarding to the appropriate GP as required Undertake vaginal swabs where necessary and initiate treatment with liaison with GPs/ANP Assistance in Minor Operations and IUCD fitting with Partners if necessary Carry out ECG, taking bloods and preparing other laboratory samples Second line monitoring of bloods, Rheumatology Assist in the training and supervision of HCA "}
