Main Purpose of the Post Responsible for providing a comprehensive range of nursing care in the GP practice including the development, implementation and evaluation of care and/or treatment programs for patients with established long-term conditions, working independently in accordance with practice protocols. Provides a wide range of general clinical care and chronic disease management reviews to practice population through the Year of Care initiative. The Practice Nurse is responsible for the care delivered, demonstrating skills of assessment and decision making in the day-to-day management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures.
