Work independently, following procedures and treatment plans, but under the direction of the patient flow hub, and within agreed boundaries. Responsible for ongoing/complex planning in regards to process mapping patients journeys for modelling purposes. Provide training on systems to new staff joining the team. The role is central to the co-ordination of both complex and non-complex discharges at Llandrindod Wells Community Hospital, and part of the Patient Flow team. The DLO will have responsibility for overseeing non-complex and complex patient discharges within PTHB Community Hospitals. Facilitating and/or assist with effective,efficient, and timely discharge of patients from hospital. Emphasis is placed upon quick turnaround of patients.This may include complex multi-disciplinary assessment signposting, liaising directly with patients and their families/ Care Transfer Coordinators/ MDT's assisting in case conferences for patients with complex health and social care,palliative care or rehabilitation or housing needs. Work as part of the flow hub and work effectively with the wider multi-disciplinary team and third sector groups to facilitate patient discharges in a timely manner across Powys Supporting Ward staff and multi-disciplinary teams in facilitating effective, safe, and timely patient discharge.
