MULTI-DISCIPLINARY
The Discharge Liaison Officer (DLO) position at Powys Teaching Health Board in Llandrindod Wells is focused on coordinating both complex and non-complex patient discharges within community hospitals. This role is essential for ensuring timely and efficient patient discharge processes, emphasizing quick turnaround and facilitating multidisciplinary assessments. The DLO will liaise with patients, their families, care transfer coordinators, and other healthcare professionals to provide support and information, particularly for patients with complex health, social care, or housing needs.

The DLO will work independently while following established procedures and treatment plans under the guidance of the Patient Flow Hub, engaging closely with a wide range of professionals and external agencies. Responsibilities also include contributing to process mapping for patient journeys, providing training for new staff, and supporting ward teams to ensure safe and effective discharges. Proficiency in Welsh is preferred, and the position is offered on a fixed-term basis for 12 months due to funding constraints.
