To undertake holistic needs assessment, care planning and evaluation from diagnosis, through treatment and beyond. This will necessitate high levels of autonomy and advocacy ensuring the delivery of ongoing appropriate care to ensure individuals have a good quality of life. Lead on the development, implementation and evaluation of Nurse Led Clinics to support the delivery of Holistic Needs assessment either by telephone or face to face. To establish working relationships and communication links between primary secondary tertiary and independent care providers to improve care for patients and provide specialist clinical expertise, advice and support when required. Ensure that professionals adhere to the implementation and monitoring of relevant clinical guidelines, policies and procedures within their scope of practice and speciality.
