The post requires a high level of clinical expertise which has been developed through experience in practice. We are dedicated to supporting continuing professional development and provide excellent leadership through mentorship, coaching and education for all grades of staff, to provide high quality patient care in line with Northumbria Healthcare Trusts Vision. As a deputy team lead the post holder will support the Team Lead to establish and maintain robust communication networks with patients, carers, team members, GPs and other professionals and agencies. You will act as a role model for the Community Nursing service, our patients and families exercising accountability for patient care in line the NMC code of professional conduct. You will work with the Team Lead to role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries. Assess, plan, implement and evaluate care packages for patients on their caseload which can include the analysis of clinical, social and family situations. To exercise accountability for patient care, as set out in the NMC Code of Professional Conduct and act within the policies of Northumbria Healthcare Foundation Trust. To support the Team Lead and in their absence be responsible for the planning and delivery of District Nursing services within a designated area, prioritising clinical need to provide holistic care. To be accountable for own practice working without direct supervision. To act as an advocate to empower individuals, their carers and their families to influence and use available resources. To carry out risk assessment and management of the physical environment to safeguard patients, staff and carers Promote effective use of Trust policy regarding infection control within the nursing team, the clinical setting and patients' homes. Make referrals to other disciplines or agencies.
