This work will be carried out in collaboration with practice teams within the designated PCN. Key Responsibilities Take responsibility for the collection and safe transport of vaccine supplies and consumables from practices within the PCN, ensuring adherence to the correct governance processes. Visit housebound patients, assess their clinical suitability for vaccination, and obtain informed consent before administering the vaccine. Ensure timely and accurate data entry into point-of-care systems and update practices with any relevant information.
