DHC is looking for skilled, self-motivated nurses or healthcare professionals to administer vaccinations to patients in their homes. This role involves working closely with our friendly teams at East Surrey GP practices, who will oversee the service.
This service will help provide high-quality care, close to home, for all patients.
The post holder will be responsible for the safe and effective delivery of vaccines to patients in their homes. You will be required to collect and transport vaccines, assess the clinical appropriateness of vaccination, administer the vaccine, and record your work using a point-of-care system.
You will work closely with our GP practices and administrators and will receive all necessary statutory and mandatory training from DHC.
You must be a confident practitioner, skilled in working with frail patients, and able to manage your workload independently in a community setting.
The postholder will be responsible for the safe and efficient delivery of COVID-19 vaccines to patients in their homes across the East Surrey area. They will also ensure accurate data entry into the appropriate clinical systems and maintain effective communication with the Primary Care Network (PCN).
This work will be carried out in collaboration with practice teams within the designated PCN.
