They will work under the direction of the senior members of the IDT and in conjunction with the ward multi-disciplinary teams (MDT). They will assist with planning and implementing patient discharge; as well as aiding general flow of patients through the hospital. Please note that this vacancy may close early due to the amount of application the vacancy receive. An exciting opportunity has arisen for Patient Flow Coordinator in the integrated discharge team working across both sites of Mount Vernon and Hillingdon Hospital. This role will require a dynamic approach to discharge planning and processes that will ultimately improve the length of stay to the patient and will support safe and time discharges.
