The Patient Flow Coordinator at The Hillingdon Hospital NHS Foundation Trust is a vital member of the Integrated Discharge Team (IDT), focused on enhancing patient care through effective discharge planning and hospital patient flow management. This ward-based role involves collaboration with senior IDT members and multidisciplinary teams to ensure timely and safe discharges, thereby improving patients' lengths of stay.
