To provide and oversee a Clinical Pharmacy Service to the four ECCH Primary Care Home Teams and beds with care focusing on delivering safe, evidence-based, cost effective, high-quality pharmaceutical aspects of the Primary Care Home Team Service. To support the provision of Clinical Pharmacy services to the Community Hospital ward and specialist palliative care beds. To explore service development/expansion in conjunction with the Head of Pharmacy where opportunity arises. To ensure that there is compliance with medicines legislation; national and local guidance and to manage risks associated with medicines use. To ensure appropriate governance is in place regarding medicines, including leading on the response to any drug safety alerts or changes in clinical guidance and escalate as appropriate. To provide professional expertise, advice, support and training on medicines management policies and medicines optimisation to health professionals and directly manage the PCH clinical pharmacy technicians. To organise and participate in regular training and feedback sessions as appropriate.
