We have an exciting opportunity for an enthusiastic and proactive prescribing pharmacist to join our team, delivering and leading innovative pharmacy services to the community.

As a leader in this organisation, no two days will be the same; you will have patient contact; you will work as part of a multi-disciplinary team; you will problem solve and you will have the opportunity to shape the pharmacy service you lead and deliver.

Prospective candidates should be efficient, enthusiastic and confident to work autonomously within a supportive team and their sphere of competence. They should have excellent communication skills as the job will involve liaising with social care, clinical and community teams as well as patients and their carers.

You will have a desire to improve patient outcomes; an aspiration to participate in research, the will to learn and develop and the opportunity to influence the future of pharmacy services.

You will have an MPharm degree or equivalent, independent non-medical prescribing qualification and be registered with the GPHC.

You will need to be flexible in your approach to work and able to travel as the role involves hybrid working.

Part time hours will be considered for suitable candidates.

Consideration would be given for an experienced band 8a development role for those looking to step up to an 8b with time and support.

You will be working and leading as part of a multidisciplinary team supporting our Community Hospital ward (Minsmere) and our Four Primary Care Home teams deliver patient centred care primarily. You will have the opportunity to provide clinical input into our bedded rehabilitation and specialist palliative care unit.

To provide and oversee a Clinical Pharmacy Service to the four ECCH Primary Care Home Teams and beds with care focusing on delivering safe, evidence-based, cost effective, high-quality pharmaceutical aspects of the Primary Care Home Team Service.

To support the provision of Clinical Pharmacy services to the Community Hospital ward and specialist palliative care beds.

To explore service development/expansion in conjunction with the Head of Pharmacy where opportunity arises.

To ensure that there is compliance with medicines legislation; national and local guidance and to manage risks associated with medicines use.

To ensure appropriate governance is in place regarding medicines, including leading on the response to any drug safety alerts or changes in clinical guidance and escalate as appropriate.

To provide professional expertise, advice, support and training on medicines management policies and medicines optimisation to health professionals and directly manage the PCH clinical pharmacy technicians.

To organise and participate in regular training and feedback sessions as appropriate.

About us

To provide and receive complex patient information and agree therapeutic outcomes with clinicians including prescribing and requesting investigations where appropriate.

To provide a high-quality Clinical Pharmacy Service including individual patient medicines review, counselling, medicines reconciliation and full clinical screening of prescriptions to ensure safe and cost-effective use of evidence based and rational use of medicines, in line with local policies and national guidance ensuring medicines are optimised and risks to patients are minimised.

To provide drug or medicine related information, including legalities, to multiple clinicians, patients, relatives, and carers.

To review all medication incidents and co-produce mitigations (e.g., training; policies; SOPs) and share learning across the organisation.

To participate in and undertake audits as agreed including collecting and recording data for KPIs and CQUINs.

To assist in reviewing and developing medicines related policies, SOPs and guidelines.

To be responsible for the delivery of high quality, comprehensive, safe and effective pharmaceutical services to patients, clinicians and staff.

To support the Head of Pharmacy in managing and professionally leading the Pharmacy and Medicines Optimisation Teams in the organisation.

To facilitate and support the organisation in achieving compliance with the Care Quality Commission Standards and NHS Resolution standards in relation to medicines handling within the organisation.

To act as Medication Safety Officer.

To support the whole organisation with medicines related advice.

Be willing and able to travel to various sites within the organisation to ensure delivery of the pharmacy service

To support the training, education and development of pharmacy staff and other clinicians as appropriate, including those undertaking educational training programmes.
