The Enhanced fall service occupational therapist will work alongside an ACAH Acute Care Practitioner responding to falls and sick patients in the community, providing urgent Holistic assessment of mostly unwell elderly patients at home. The successful candidate will have previous NHS & experience working in emergency portals/community services or have a range of transferable skills including assessing falls patients. The role will have flexible 7 day working. Falls service operational service times would be currently from 0900 to 2000hrs. We are keen to meet dynamic, enthusiastic and highly motivated Occupational Therapists interested in working in our supportive admission avoidance team providing the highest standards of care to keep our frail patients safe in their own homes. Our focus is reduction of ED attendances and safe admission avoidance, acute/subacute interventions to improve functional outcomes & specialist falls assessment and falls prevention. Applicants will need to hold a full driving licence as this is required for the role. A vehicle is provided to enable all necessary equipment to be carried by the team. This role works autonomously as a specialised practitioner managing the clinical delivery of care within Acute Care at Home.
