The University Hospital of North Midlands NHS Trust is seeking an Advanced Occupational Therapist for their ACAH community Enhanced Falls service in Stoke on Trent. This senior role involves working within the Urgent Community Response (UCR) Service, where the therapist will collaborate with an Acute Care Practitioner to provide holistic assessments and care for elderly patients experiencing falls and other health issues in their homes. The position requires experience in emergency or community health settings, particularly in assessing falls, and emphasises flexible working hours across a seven-day period, focusing on reducing hospital admissions and improving patient outcomes.

The successful candidate will operate autonomously, managing clinical care delivery and resource organisation within the Enhanced Falls service. They will engage with a diverse range of patients, including those with complex medical conditions. A full driving license is necessary as the role involves travel to patient locations with a provided vehicle for equipment transport.
