To assist the Practice Nurses in the annual reviews for patients on the practice register for Diabetes, Chronic Obstructive Pulmonary Disease (COPD), Asthma, Chronic Heart Disease, Chronic Kidney disease, Ischemic Heart Disease, Hypothyroidism, Mental Health and Hypertensive Disease. To assist in the smooth running of clinics ensuring all necessary equipment and stock is available and high quality standards of infection control are maintained.
