Continuing Care is a process of assessing children and adults to identify people with complex health conditions who are eligible for NHS funded care and support. The AACC admin team manages all incoming referrals and provides day to day admin support to our Nurse Assessor team. The ICB support over 3000 people a year in Bristol, North Somerset and South Gloucestershire through AACC.To meet regularly with the Administration Manager to ensure that targets are being met and to monitor staff performance. The information we deal with is of a sensitive and clinical nature and therefore accuracy, attention to detail and confidentiality for us is of paramount importance. Successful applicants will be computer literate with competent typing skills. They will be confident, quick learners, and be able to use their own initiative. They will have excellent communication and good time management skills, with the ability to achieve high standards of excellence within their work.
