The majority of these will be unexpected diagnoses so patients may require advice and support to encourage them to enter care. The post holder will work with the health advisor team and clinical leads to ensure patients are receiving their results efficiently and they will be expected to provide health education, advice and support to patients following their diagnosis along the patient pathway into care. Manage the patient pathway for all patients with reactive blood bourne results who were tested in the ED department. provide support and health education for those patients identified as having a positive BBV result. Co-ordinate the scheduling of appointments for patients requiring attendance at Sexual Health Hounslow clinics. co-ordinate the tracking of the diagnostic/treatment pathway for patients with a reactive BBV result. Reporting if patients are being assessed in accordance with national targets for timeliness of treatment. Take a lead role in the organisation and management of the HBV and HCV MDT within Sexual Health Hounslow and the wider trust, ensuring the necessary documentation and results are available for discussion. To develop and maintain supportive and effective relationships with the ED team and the Hepatology teams in the hospital. To work with other service providers to re-engage people living with BBV into appropriate care. Liaise with patients to encourage them to enter care and to manage their pathway. Appropriately remind clients of their appointment to facilitate care where needed. This may include sending reminder letters or contacting patients by telephone, or text. To lead the development and implementation of contact tracing for patients with a positive BBV result if necessary, developing protocols and materials to support this project. Education and Training To support the provision of education and support to clinicians in the ED to ensure a high uptake of BBV testing within the department. Utilise reports of testing number in ED to identify specific training needs across the ED departments. To provide regular updates regarding the project to Sexual Health Hounslow and Hepatology department. Data and Reporting To undertake regular audits of BBV testing within the ED. Data collection and regular reporting of the linkage into treatment for patients with a positive BBV test. To be responsible for tracking monitoring/treatment pathway for BBV patients. Monitoring whether patients are being treated in accordance with any local or national targets for timeliness of treatment. Collate reports and updates for the clinical leads when requested. Training and personal development To employ a pro-active approach to good team working, including being able to cover the responsibilities of colleagues in similar roles within the health advisor team and across site where needed. To collaborate with clinical lead, senior health advisor and Hepatology teams to promote service improvement and development. To attend relevant meetings and represent data quality as requested. To ensure skills and knowledge are up-to-date and relevant to the role. To follow relevant Trust policies and professional codes and to maintain registration where this is a requirement of the role. To participate in planned training events and to attend all mandatory training sessions as requested.
