Due to an exciting change to the service we will be offering training to be based in Wells at Priory House, and then once induction period has ended base will be in Ashford Court Taunton. The Somerset Hub for Coordinating Care consists of the Intermediate Care Bed Hub, Somerset Primary Link Urgent Referral Service, Hospital at Home Remote Monitoring Service and End of Life Care Coordination Centre. We are a friendly, hard working team and we heavily focus on health and wellbeing. This job role will predominantly be based within the Somerset Primary Link Team but successful applicants will have the opportunity to work across the teams. You'll have strong attention to detail, advanced multitasking skills and will be comfortable working within a busy working environment. Our service hours are eight AM to nine thirty PM, 7 days a week, every day of the year and our service does not close. We are also open on all bank holidays. You'll work closely with the Somerset Hub for Coordinating Care clinicians in facilitating a smooth referral process for service users and patients, providing a single point of contact for information on patients to be transferred / admitted to community hospital beds within the Primary Care Trust. You'll also support the Somerset Hub for Coordinating Care clinicians in providing an efficient referral process, to include booking transport and liaising with patients. You will be required to work across multiple computer system's, coordinate telephone calls and completing a range of reports.
