As part of the Somerset Hub for Coordinating Care, you'll play a vital role in ensuring patients receive the care they need seamlessly, whether it’s assisting with referrals or booking transport. Our dynamic team values wellbeing and collaboration, and we offer training to equip you for success. You’ll primarily be working within the Somerset Primary Link Team, with opportunities to branch out across various services, including the Intermediate Care Bed Hub and Hospital at Home. With shifts spanning from 8 AM to 9:30 PM, across all days of the week, including bank holidays, you can find a role that fits your lifestyle.
