You will be based within our (CMHT) and will be an integral part of the Multidisciplinary Team and help to embed clinical pharmacy services. Your skills and experience will be pivotal in supporting this by ensuring pharmaceutical care and medicines optimisation are fully implemented to the highest standards. If you are keen to learn, looking for a rewarding career at a time of change that offers opportunities to develop your role then this role is for you The Clinical Pharmacy Technician is responsible for ensuring community patients are prioritised appropriately and high risk patients are identified to ensure the patient receives best outcome around safe and effective use of medicines while in our service. This includes : Identify individuals prescribed high risk medicines (clozapine, lithium, sodium valproate & high dose antipsychotics) and completion of required physical health and side effect monitoring Screen physical health monitoring in line with national guidance (NICE, Lester tool)the review of currently prescribed medicines following transitions of care Work with the local Medicines Optimisation (MO) team in developing and delivering the clinical pharmacy service to ensure safe, therapeutic and cost effective use of medication in community teams To provide medicines optimisation training to staff re assessment, storage, use and disposal of medication in accordance with trust requirements The post holder will supervise any medicine optimisation technician in the future.
