You will be based within our (CMHT) and will be an integral part of the Multidisciplinary Team and help to embed clinical pharmacy services. This includes : Identify individuals prescribed high risk medicines (clozapine, lithium, sodium valproate & high dose antipsychotics) and completion of required physical health and side effect monitoring Screen physical health monitoring in line with national guidance (NICE, Lester tool)the review of currently prescribed medicines following transitions of care Work with the local Medicines Optimisation (MO) team in developing and delivering the clinical pharmacy service to ensure safe, therapeutic and cost effective use of medication in community teams To provide medicines optimisation training to staff re assessment, storage, use and disposal of medication in accordance with trust requirements The post holder will supervise any medicine optimisation technician in the future.
