Preventing hospital admission Promoting a greater understanding of diabetes and empowering patients, their families and carers and other agencies and professionals involved with the patient through education, support and counselling. Nurturing collaboration and shared decision making Alongside a senior diabetes specialist nurse, you will be responsible for supporting a caseload of patients with complex diabetes management needs, using evidence based practice and patient centred goals to assess, plan, implement and evaluate interventions. Working as part of a wider Multi-Disciplinary Team, you will be involved in the assessment, review and complex care planning, employing the use of diabetes technologies, where appropriate, for people with complex diabetes management needs, in order to provide seamless, integrated care. Representing diabetes specialist nursing team and attending multidisciplinary meetings and diabetes clinics in hospital and the community Conducting patient reviews in a wide range of settings, depending on service and clinical needs.
