The Wye Valley NHS Trust is seeking a Diabetes Specialist Nurse to join their diabetes specialist nursing team in Herefordshire. This role, available as a band 5/6 development position, offers a supportive environment for individuals interested in developing their expertise in diabetes care, even if they lack prior experience in the field. The successful candidate will work part-time (28 hours per week) within a multidisciplinary team, focusing on providing high-quality, individualized care to patients with complex diabetes management needs, while also preventing hospital admissions and enhancing understanding of diabetes through education and support for patients, families, and other professionals.

The position involves managing a caseload of patients with complex needs alongside a senior diabetes specialist nurse, using evidence-based practices to plan and evaluate care interventions. The role includes conducting patient reviews in diverse settings, delivering structured education sessions, and participating in multidisciplinary meetings and clinics.
