The post offers the opportunity to work as a screening practitioner within our Older Adults Community Mental Heath Team. Training/shadowing opportunities will be provided. There is the opportunity for professional development within this role. Regular Management and Clinical Supervision is provided. Applicants must have full UK NMC registration An opportunity has arisen for a qualified and experienced individual to join the busy and vibrant later Life and Memory Service pathway in Wigan on a 12 month fixed term/secondment basis to cover maternity leave. The pathway comprises of the memory assessment and treatment service, the care home liaison team and our older adults CMHT.The successful candidate will be required to fulfil the role of a CPN as part of a multi-disciplinary team which operates from Claire House Health Centre in Wigan. You will be responsible for screening and completing initial assessments for any new referrals in to the community mental health team. You will also be responsible for managing a case load of clients with functional and organic mental health issues and associated social care issues, for which risk assessment and risk management is an essential requirement. In addition you may also assess people with memory problems and offer support following diagnosis.
