The successful applicant will provide professional, holistic nursing care and support to patients, carers and families within a variety of community settings, delivered with compassion, dignity and respect. This post will require individuals that can deliver the highest quality care, ensuring evidence based/ best practice and has a commitment to continuous professional development. Applicants to have excellent communication skills and have a flexible and adaptable approach to changing work patterns. Variety of shift patterns over a 7 day service.We offer mentorship, working life balance and exciting professional development opportunities to meet your needs. The successful candidates will be required to function as a member of the multi-disciplinary team, to assess individual patient's care needs and escalate concerns to the correct member of the team. Working as a lone worker responsible for carrying out the delegation of tasks, assessments and evaluations, and escalating concerns as appropriate. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
