This is a 37.5-hour fixed-term post for 12 months to cover maternity leave, based within the Crane Valley team. The role of the Community Staff Nurse has evolved over the years, and we now care for complex patients in their home environments. The skills required for Community Nursing include complex wound management, palliative care, end-of-life care, medicines management, IV drug administration as well as having a high standard of holistic assessment and care planning skills. Training is available within the Trust for the successful applicant. Our team works closely with members of the wider multi-disciplinary teams, including local hospices, Complex Care Nurses, Health and Social Care Coordinators, Specialist Nurses and Therapist colleagues. The team is also supported by the Intermediate Care Service and utilises our community hospitals for patient rehabilitation. To assess, plan, implement and evaluate treatment and care to people on an allocated caseload; promoting independence and autonomy; working within a multi-disciplinary team. Advance own clinical knowledge, skill and competence based on current evidence through educational programmes Provide advice to others regarding the management and care of patients/service users To demonstrate clinical effectiveness by use of evidence based practice and outcome measures.
