An opportunity has arisen for a dynamic Registered Nurse to join the District Nursing Service covering East Dorset. This is a 37.5-hour fixed-term post for 12 months to cover maternity leave, based within the Crane Valley team.
The role of the Community Staff Nurse has evolved over the years, and we now care for complex patients in their home environments. The skills required for Community Nursing include complex wound management, palliative care, end-of-life care, medicines management, IV drug administration as well as having a high standard of holistic assessment and care planning skills. Training is available within the Trust for the successful applicant.

Our team works closely with members of the wider multi-disciplinary teams, including local hospices, Complex Care Nurses, Health and Social Care Coordinators, Specialist Nurses and Therapist colleagues. The team is also supported by the Intermediate Care Service and utilises our community hospitals for patient rehabilitation.
To assess, plan, implement and evaluate treatment and care to people on an allocated caseload; promoting independence and autonomy; working within a multi-disciplinary team.
Advance own clinical knowledge, skill and competence based on current evidence through educational programmes
Provide advice to others regarding the management and care of patients/service users
To demonstrate clinical effectiveness by use of evidence based practice and outcome measures.
Plan, implement and review health improvement programmes in a range of settings
Recognise, assess and manage risk across the immediate and wider working environment ensuring statutory requirements are met
To be responsible for patient safety through knowledge of systems, legal requirements and understanding of litigation
To communicate effectively in verbal and written form in the exchange of complex, sensitive or contentious information in difficult situations using de-escalation, mediation, resolution and professional Duty of Candour
To evaluate care, taking appropriate action leading to improvement in quality standards through clinical audit, and contributing to root cause analysis and dealing with complaints
