The purpose of the role is to provide a positive patient experience at the time the patient is identified as being medically fit for discharge. The post holder will work as part of a team to provide a safe, effective and efficient care environment for patients and their carers with a specific focus on safe and timely discharge from hospital. They will also be a point of contact for patients following discharge. The post holder will be responsible for facilitating the prompt discharge of 4th cardiothoracic surgical patients from the floor, through early identification of the patient being deemed medically fit for discharge and having all their discharge documentation and medications prepared in a timely manner. The post holder will provide the necessary advice and guidance to patients in relation to their safe discharge and on-going recovery. The post holder will also be responsible for contacting patients two - three days post discharge for a welfare check. The post holder will be integral to developing this role to ensure continuity and high standards of care ensuring patients are prepared pre-discharge as per the integrated care pathway and discharge care bundle. A secondary outcome of this role will be improved utilisation of bed capacity. Early discharges from the ward will enable a more efficient flow of patients from the intensive care and high dependency areas.
