Barts Health NHS Trust is seeking a motivated and enthusiastic nurse to join their Discharge Lounge team for Cardiothoracic surgical wards at St Bartholomew's Hospital in London. The role involves caring for patients who have undergone cardiac and thoracic procedures, ensuring a positive experience as they transition from hospital to home. The successful candidate will have a degree of autonomy in identifying patients medically fit for discharge, managing their documentation and medications, and providing guidance for their ongoing recovery. The position is geared towards enhancing patient flow and bed capacity utilisation, supporting timely discharges, and offering post-discharge welfare checks.
