You will also work with other health professionals and healthcare assistants to raise awareness and educate staff, patients and relative about the of importance of promoting independence and prevention of deconditioning during hospital admissions. To work directly with patients to provide interventions such as exercise programmes, practice self-care and other activities of daily living. You will also provide education and support to patients, relatives and other health care staff on the importance of promoting independence and preventing deconditioning during their hospital stay and beyond. To assess and manage delegated patients with mobility and occupational difficulties, reporting back to the registered practitioner as necessary.
