We are looking for dedicated individuals who are committed to transforming community mental health services for our aging population. You will be working alongside a well established MDT including, Social workers, Mental Health nurses, psychologists, pharmacists and many other AHP's who will support you in delivering excellent holistic care. The post is also aligned to a thriving network of Care Community groups that support each other to tackle health inequalities, pool collective knowledge and services whilst also streamlining access to our respective areas. You will be responsible for the delivery of a range of therapeutic interventions and other aspects of the service user's care plan under the guidance of a registered practitioner and receive regular support to accomplish this. Community Support Worker will be responsible for the delivery of care and support for service users within the community mental health setting and reporting on the service users wellbeing under the guidance of a registered mental health practitioner. You will also be responsible for the delivery of a range of therapeutic interventions and other aspects of the service user's care plan under the guidance of a registered mental health practitioner.
