The post holder will be supporting our newly developed frailty assessment unit and advocating an 'home first' approach from hospital. The aim of the service, along side the Front of House team (Tocals) is to provide seamless care to patients as they transition between acute and community sectors. We work together to provide early intervention, period of rehabilitation and assessment to reduce the risk of loss in function, risk of deconditioning, nutritional deficit, deterioration in their mental health to increase people's time spent at home rather than hospital. Our approach is to deliver patient care in a holistic, person-centred manner, supporting patients, when possible, to stay/return in a safe manner to their own homes where they may continue to be a valued member of their own community. This would included outreaching into community settings. This role involves supervising students, junior members of staff, rehabilitation assistants and therapy support staff (assistant practitioners and support workers). The post holder will be required to participate in the provision of on call respiratory service and future developments in 7 day working. Successful applicants will have experience of working with people who have Frailty syndrome & enjoy working as part of a multi professional team within the acute and community setting. You will work alongside acute and community colleagues and services to create a joined up rehabilitation pathway and positive experience for our population. The post holder will require broad clinical experience in frailty & will need to provide specialist assessments and formulate appropriate treatment plans using in depth clinical reasoning with a focus on patient-centred care, current HB pressures and awareness of patient flow. This role could be part of a rotational opportunity between Front of House, stroke & general rehabilitation within Prince Philip Hospital. Successful applicants will require an excellent ability to work autonomously, in high pressure and fast paced environments, managing and prioritising a diverse and unpredictable caseload, while keeping the HB values central to everything they do. As a versatile postgraduate, you will need to have strong clinical grounding in older people rehabilitation, expertise in frailty syndrome, stroke, neurological conditions, cardio respiratory conditions, MSK problems and a good understanding of current issues affecting acute, intermediate care and community services.
