Hywel Dda University Health Board is looking for an experienced Specialist Physiotherapist to join their Frailty Service in Carmarthenshire. The role involves supporting a newly established frailty assessment unit with a focus on a 'home first' approach to patient care, enabling seamless transitions between acute and community settings. The successful candidate will provide holistic, person-centered treatment to enhance rehabilitation and reduce risks such as deconditioning and nutritional deficits, while also collaborating with a multidisciplinary team. Additionally, the position includes supervising junior staff and supporting on-call respiratory services.

Candidates should have extensive clinical experience in frailty syndrome and be capable of managing a varied caseload autonomously in a high-pressure environment. A strong background in rehabilitation for older adults and familiarity with related conditions such as stroke and neurological issues are essential. The ability to speak Welsh is desirable but not mandatory, as the Health Board supports staff in achieving bilingualism. Flexibility for travel within the county and across the health board may be required.
