The post holder will provide specialist nursing support to patients with Colorectal Cancers and willbe an integral part of the multi-professional colorectal cancer team The post holder will be mentored and supported by the Colorectal Cancer Nurse Specialist Nursing team and will carry a supervised caseload providing specialist nursing advice, information and support to patients with colorectal cancers and their families at all stages of their disease, and, in particular whilst they are being cared for at South Tees NHS Hospitals Foundation Trust thus ensuring a smooth transition of care from primary surgery through to radiotherapy/chemotherapy and into the community. Strong links will be expected to be maintained across the surgical and oncology pathway to ensure seamless care for the patient. The post holder will liaise with services within South Tees Hospitals NHS Trust and with other key partners such as community teams, social services, hospices and voluntary agencies to ensure quality service provision for all patients under their care. They will contribute to improving clinical practice, enhancing service provision and streamliningpatient focused care. The post holder will be an active member of the multi-disciplinary team and network with other professionals, locally and nationally for the purpose of contributing to developing practice, research and audit, development of standardised patient information, and clinical supervision.
