On both wards there is daily input from the mental health team for each patient. The role of the post holder would be to support the team (ACPs in particular) with the management of common medical condition such as heart failure, renal impairment, COPD, and diabetes. To provide medical cover to Alderney Hospital Inpatients. The doctor will provide medical care for the patients on the wards, in collaboration with the Responsible Clinician/Multi-Professional Advanced Clinician, Advanced Practitioner, and the ward nursing team The post holder will support the supervision of the clinical work of the Advanced Practitioner and will develop a mutually supportive clinical relationship with the Advanced Practitioner in managing the patient's clinical need The postholder will collaborate with the multi-disciplinary team in the planning and provision of care, investigations, treatment, and discharge for patient The postholder will provide timely clinical review of patients, participate in ward rounds, and keep up-to-date and accurate clinical record The postholder will implement appropriate treatment plans for patients, in collaboration with the patient, and the multidisciplinary team, including relevant assessment of treatment escalation plans and resuscitation status where appropriate The postholder will carry out the daily tasks as identified in the Medical Diary and in liaison with the nurse in charge of the ward.
