As a valued member of the FUSE Team, you will be contributing to the NHS Long Term Plan to improve public health, de-escalating emergency admissions and reducing pressures on NHS resources. In collaboration with multi-agency teams, stakeholders, and voluntary organisations you will be working as a key member of staff to sign post and support to access local services and activities to improve their health and wellbeing for clients with complex needs. FUSE values a strength-based approach to meeting holistic needs and aspirations of clients, reaching beyond medical, psychological, and social needs. You will contribute to public health research and service innovation by conducting research interviews to ascertain the FUSE experience for clients. This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility. Please note that Livewell may close the job advertisement earlier than the specified deadline if a high number of applications are submitted.
